Doctors at Kamineni Hospitals gave a new lease of life to a 45-year-old woman from Vijayawada, who was diagnosed with a rare case of fibroid red degeneration.She was brought to Kamineni Hospitals, Vijayawada, with a complaint of pain in the lower abdomen for two months.

The patient was suffering from low-grade fever occasionally.However, it would subside spontaneously.

The pain in the abdomen was not related to the menstrual cycle.It was being relieved to some extentwith analgesics.

Her menstrual cycles were regular, and the flow was moderate.She underwent 2 cesarean deliveries.

On evaluation with ultrasound abdomen and pelvis, we found.

Doctors found a small 4*3 cm anterior wall intramural fibroid with no other pathology.

On high-dose contrast-enhanced computed tomography (CECT), multiple tiny renal calculi were noted along with the fibroid.Doctors proceeded for a total laparoscopic hysterectomy after counseling the patient.

Doctors found omental adhesions to the anterior abdominal wall which were released, and Tubes and ovaries were healthy on both the sides.

On examining the cutsection of specimen, we could find some areas of hemorrhage inside the fibroid.On histopathological examination, red degeneration was confirmed.Her postoperative period was uneventful and she had 100% pain relief thus attributing the pain retrospectively to the red degeneration of fibroid.

Thus the moral of the story (lesson learnt) is- In nonpregnant women with fibroid uterus having mild grade fever and pain abdomen, possibility of red degeneration should be kept in mind though it’s not so common.

Fibroid (leiomyoma) of the uterus is the most common benign condition found in women of reproductive age group.

It may be asymptomatic or can be present with a wide variety of symptoms like menstrual complaints, lump in the abdomen, pain in the abdomen, urinary complaints, pressure symptoms and anemia.Fibroids may undergo various types of degenerations, such as hyaline, calcareous, fatty, myxomatous and red degenerations.

Red degeneration of fibroid (carneous degeneration) is rare — at an incidence of 3% — and it has been mentioned in medical literature that it occurs during pregnancy but is rarely found in non-pregnant women.Not all the fibroids need to be removed, nor all the fibroids should be neglected.
